2) The Quantity of the DVDs.
3) Your Full Name. 
4) Your Membership Code.
5) Your Phone Number.
6) Your Address. 
* Please take note that the DVDs will be sent to the branch that is nearest to you, so you would need to collect the DVDs from there.

Once we receive your order, we will contact you via email or telephone call and give you a Maybank Account Number for you to bank in the payment for the DVDs.
